There are some bad apples in every barrel but possibly the biggest scam is baked into most fee-for-service health systems. The more you do, the more you get paid …. so you tend to do more, whether it is necessary or not.
And even without any intent to defraud, this tendency is strengthened by adversarial legal arrangements which lead to defensive medicine, where unnecessary investigations are carried out to cover the doctor legally rather than for the benefit of the patient.
